Students applying for a permit to stay for study ("Permesso di soggiorno") should prepare the following documents for the Italian police:

one copy of your bank statement or grant document with amount expressed in Euro that proves you can support yourself during your study period in Italy
one copy of your health insurance with clear coverage explanations (period and country of validity) for non-EU citizens and E128 or E111 form or European Health Insurance Card (which has already replaced the E128 and E111 forms in most EU countries) for EU citizens.
one copy of housing contract if available

The Permit must be requested only to the Questura di Milano:
Immigration Office
via Montebello 28 tel. 02.62261 or 02.62263400

Please note that non-EU students usually have to stand long queues at the immigration office, for this reason we suggest they go there very early (6 am) to get an appointment.
